Late Soccer Registration Date:
A late walk-in registration signup will be held at Mountain Mikes Pizza:
          June 3       6:00pm -  8:00pm       Tuesday

Late Registration (Late Registration begins May 20th, 2008)
If you fail to turn in COMPLETED registration by May 19th you are considered LATE.

1. The registration fee is increased to $90.00 per player.
2. You forfeit your chance to request a particular coach or team.
3. Late registration may be available until July 1st, but the later a player registers, the less chance they will have of being placed on a team.
4. After July 1st, only families moving into the area will be allowed to register.
5. Email the phsoccer Registrar or leave message 878-4463 for LATE registration procedures.

NOTE:
Only Completed registrations will be accepted (see Registration Requirements, below).
Completed registrations are date stamped on arrival to the registrar. 
Teams are filled on a first come, first serve basis.

Registration Requirements (what to include with the registration forms)

1. Current (within last year), nonreturnable, wallet sized color photo (cut to 1" by 1", name on back).
2. $90.00 Registration fee per player. Memory mate photo is included in the registration fee.
3. If your child is playing soccer for the first time with PHYSC, you must include a nonreturnable copy of their original birth certificate.  Hospital or baptismal copies are NOT acceptable.
4. Name and phone number of person to contact in case of emergency (NOT parent or guardian).
5. Doctor / medical contact name and phone number.
6. Parent/guardian signatures in BOTH places on the Registration form
7. Separate checks for registration and uniform purchase (if new uniform needed) payable to PHYSC.

NOTE: It is possible to play up in an older age group (except U6), but you will not be placed on a team unless all players at that age group have already been placed on a team (coaches permission also required).  Playing down is prohibited.

Refund Policy

• A full refund will be given to players who move out of the area before the games begin or who are injured before the games begin. 
• A 50% refund will be given to players who choose not to play for other reasons before games begin. 
• No refunds will be given to players who drop after the first day of games.
