Soccer Registration Dates:
Registration will be held at Placer Hills School Rooms 9 and 10:
May 9

Mailing in forms is not accepted. You must attend one of the above registrations.
Only Completed registrations will be accepted (see Registration Requirements, below).
Completed registrations are date stamped on arrival to the registrar.
Teams are filled on a first come, first serve basis. Registration Requirements (what to bring with you)
- Current (within last year), non-returnable, wallet sized color photo (cut to 1" by 1", name on back). Will not register without photo.
- $75.00 Registration fee (third child reduced to $50.00). Memory mate photo is included in the registration fee.
- If your child is playing soccer for the first time with PHYSC, you must bring a non-returnable copy of their original birth certificate. Hospital or baptismal copies are
NOT acceptable.
- Name and phone number of person to contact in case of emergency (NOT parent or guardian).
- Doctor / medical contact name and phone number.
- Separate checks for registration and uniform purchase (if new uniform needed) payable to PHYSC.
- A Parent/guardian must attend registration and sign in BOTH places on the Registration form

Uniform orders will be taken at registration. Remember a separate check will be required. Visit our Uniform page
for price information. Late Registration (Late Registration begins May 31st, 2009)
You are considered LATE if you fail to turn in
COMPLETED registration by May 30th (must be postmarked on or before May 30th).
- The registration fee is increased to $100.00 per player.
- You forfeit your chance to request a particular coach or team.
- Late registration may be available until June 29th, 2009, but the later a player registers, the less chance they will have of being placed on a team.
- Beginning June 30th, only new families moving into the area will be allowed to register.
- Email the
phsoccer Registrar or leave message 878-4463 for LATE registration procedures.
Team Requests
PHYSC makes it a priority to place a player with the same coach (team) as the previous year, if so requested. Please be aware that coaches sometimes change age groups and
not all players will be eligible to return to the same coach. Every attempt will be made to accommodate player requests, but there is no guarantee
of being placed on a particular team. The date of your registration may be used to help decide team placement, so register as early as possible. You should hear from a coach in the beginning of August.

NOTE:
It is possible to play up in an older age group (except U6), but you will not be placed on a team unless all players at that age group have already been placed on a team (coaches permission also required).

- A full refund will be given to players who move out of the area before the games begin or who are injured before the games begin.
- A 50% refund will be given to players who choose not to play for other reasons before games begin.
- No refunds will be given to players who drop after the first day of games
